El grupo RedFastER de la Sociedad Española de Farmacia Hospitalaria (SEFH) ha creado la app UrgRedFasterFH para servir de ayuda a los farmacéuticos
especialistas que trabajan en los servicios de urgencias de los hospitales.
El estudio EVADUR (Emergencias 2010; 22: 415-428) puso de manifiesto que el 12% de los pacientes visitados en urgencias sufre algún tipo de incidente o efecto adverso y que
la segunda causa más frecuente es la medicación (37,5%). El grupo RedFastER detectó que un 79% de los pacientes en urgencias presentan discrepancias entre la medicación que
realmente toman antes de acudir a urgencias del hospital y la que figuraba registrada en las historias clínicas. Además, la interrupción brusca de alguno de los medicamentos
puede provocar un síndrome de retirada, una exacerbación de la patología subyacente, empeorando el estado del paciente y/o complicando el cuadro agudo que le hizo acudir a
urgencias o incluso generar un nuevo problema de salud. Pero también el mantenimiento de alguno de los medicamentos, en determinadas circunstancias (fallo renal, fallo
hepático, sangrado, depresión respiratoria, intoxicación, etc.) puede suponer un riesgo para el paciente, por lo que deben ser interrumpidos temporalmente hasta estabilizar
al mismo.
La denominada conciliación puede resolver estos problemas. Se define como el proceso formal y estandarizado de obtener la lista completa de la medicación previa de un
paciente, compararla con la prescripción activa, y analizar y resolver las discrepancias encontradas. Organizaciones internacionales como la Joint Commission on the
Accreditation of Healthcare Organizations, la Organización Mundial de la Salud, la National Institute for Health and Clinical Excellence o la National Patient Safety Agency
contemplan la conciliación entre los objetivos para mejorar la seguridad de los pacientes.
La “Guía para la Conciliación de los Medicamentos en los Servicios de Urgencias” del grupo RedfastER incluye medicamentos o grupos de medicamentos ordenados por patologías.
Para cada uno de ellos se ha tenido en cuenta:
- Tiempo máximo que debe transcurrir para llevar a cabo la conciliación.
- Presencia o no de síndrome de retirada.
- Manejo en urgencias.
Esta guía en su formato en papel fue premiada en Las Mejores Iniciativas de la Farmacia del año 2011 por Correo Farmacéutico en el apartado de Atención farmacéutica y
Educación Sanitaria (Marzo 2012). En Junio de 2014 lanzamos la primera versión de la app Conciliación de Medicamentos. Además de facilitar la accesibilidad a los contenidos
de la guía en papel, permite tomar notas, buscar medicamentos, acceder rápidamente a los medicamentos que requieren una conciliación en menos de 4 h, seleccionar favoritos,
etc.
En Julio de 2019 lanzamos la versión 2.0 de la app, a la que le hemos cambiado el nombre por UrgRedFasterFH. Además de la guía de conciliación, incluye "50 Fármacos de
Urgencias", basada en la obra "50 Drugs every emergency physician should know" publicada por la American Academy of Emergency Medicine Resident and Student Association.
Constituye una guía de consulta de los fármacos más usados en urgencias. La adaptación que ha hecho el grupo RedFastER, pretende transmitir la información mínima que se
requiere conocer para poder realizar una atención rápida y eficaz en los Servicios de Urgencias. Cada una de las 50 fichas de los medicamentos tiene 3 apartados:
- Mecanismo de acción.
- Indicaciones utilizadas en urgencias y su dosificación.
- Cuándo te causará problemas.
Se trata de una guía generalista y algunos de los pacientes que se presenten en el día a día no se podrán ajustar a las condiciones aquí expuestas. Habrá que tener especial
cuidado con ellos y realizar una atención más individualizada.

bleeding, respiratory depression, poisoning, etc.) can pose a risk to the patient, so they should be temporarily interrupted until stabilized.
The so-called conciliation can solve these problems. It is defined as the formal and standardized process of obtaining the complete list of a patient's previous medication,
comparing it with active prescription, and analyzing and resolving the discrepancies found. International organizations such as the Joint Commission on the Accreditation of
Healthcare Organizations, the World Health Organization, the National Institute for Health and Clinical Excellence or the National Patient Safety Agency contemplate the
reconciliation between the objectives to improve the safety of patients.
The "Guide for the Reconciliation of Medicines in Emergency Services" of the RedfastER group includes medicines or groups of medicines ordered by pathologies. For each of
them, the following has been taken into account:
- Maximum time that must elapse to carry out the conciliation.
- Presence or not of withdrawal syndrome.
- Emergency management.
This guide in its paper format was awarded in the Best Initiatives of the Pharmacy of the year 2011 by Pharmaceutical Mail in the section of Pharmaceutical Care and Health
Education (March 2012). In June 2014, we launched the first version of the Medication Reconciliation app. In addition to facilitating accessibility to the contents of the
paper guide, it allows you to take notes, search for medications, quickly access medications that require a reconciliation in less than 4 hours, select favorites, etc.
In July of 2019 we launched version 2.0 of the app, to which we have changed the name to UrgRedFasterFH. In addition to the conciliation guide, it includes "50 Drugs of
Emergency," based on the work "50 Drugs every emergency should know" published by the American Academy of Emergency Medicine Resident and Student Association. It is a guide
for consulting the most used drugs in the emergency department. The adaptation that the RedFastER group has made, intends to transmit the minimum information that needs to
be known in order to be able to carry out a fast and efficient attention in the Emergency Services. Each of the 50 prescription cards has 3 sections:

- Indications used in the emergency department and its dosage.
- When will it cause you problems?
It is a general guide and some of the patients that appear on a day-to-day basis will not be able to adjust to the conditions set out here. We will have to take special care
with them and make more individualized attention.
